I made the heel out of chocolate candy (70% dark chocolate Apeels, 30% Brill FudgeCo.) using two heart shaped tart pans to make a mold, and then sticking the two halves together with melted chocolate: http://www.cakeswebake.com/photo/shoe-heel-mould?context=user then I carved off the excess. 

 

Then, I cut three ovals out of the cake, cantilevered them, positioned the heel, and put a dowel down through the cake at an angle and fastened the top of the dowel to the heel with more chocolate: http://www.cakeswebake.com/photo/shoe-heel-mould/prev?context=user

 

The rest is butter cream.  I iced the whole thing first in the dark  pink color, then I cut away the top of the shoe, covered it in sanding sugar, and iced over the top with the light pink.

This looks great.  How did you hold the cake together for the shoe?
