This is the "billowing" technique.  you take 2"x2" square of fondant, pleat 2 ends, pinch then trim off excess.  apply water to the back and apply to fondant covered cake.  you start at the bottom working your way up, alternating the placement on each level to fill in the empty space of the layer below. The puffs are not filled with anything. They are puffed when you pleat. once complete, you can add buttons, flowers, pearls, whatever you like in the "seams".  I add the ruffle layers last.  i put one layer at a time down until it was as full as i wanted.  i don't think you have to use 2" x 2".  you could play with smaller or larger depending on the look you were going for.  Thank you for looking.  it took a little time but was fun to do.
